Hip Pain Consultation

Hip pain can be felt in the groin, the front of the thigh, the buttock or the outer side of the hip. It can make walking, dressing and getting into a car difficult. A consultation helps find whether the source is the hip joint, the muscles or a nerve.

Hip pain: what patients usually notice

  • Hip pain while walking, standing or getting up
  • Pain in the groin, or pain near the side of the hip
  • Difficulty sitting cross-legged or putting on socks and footwear
  • Difficulty getting into a car or getting out of a low chair
  • Limping, or pain when lying on the affected side at night
  • Stiffness that makes it hard to turn the leg inward or outward

Medical terms you may hear

Hip osteoarthritis is wear and tear of the hip joint cartilage. Avascular necrosis (AVN) of the femoral head is death of part of the thigh-bone head because its blood supply is reduced. Greater trochanteric pain syndrome is pain on the outer side of the hip. A hip fracture is a break in the upper thigh bone, most often after a fall in older adults.

Common causes

  • Age-related wear of the hip joint
  • Avascular necrosis, which can follow an injury, long-term steroid use or heavy alcohol use, or sometimes has no clear cause
  • Muscle and tendon irritation on the outer hip
  • Injury or fall, especially in older adults
  • Pain referred from the lower back, which can feel like hip pain

What a consultation includes

  • A history of your pain, walking and daily activities
  • Examination of the hip, including movement, strength and walking pattern
  • X-rays of the hip where indicated
  • MRI where it will help, for example to assess early avascular necrosis
  • Advice on activity, supports and exercise

Treatment options

Hip arthritis is often managed with exercise and muscle strengthening, weight management, activity changes, walking aids where helpful, pain-relieving medicines as prescribed and injections in selected cases. Early avascular necrosis may be monitored and protected. Hip replacement (total hip replacement, THR) is discussed when symptoms are severe and non-operative care is no longer enough. Hip fractures need prompt treatment, usually surgical, at a hospital.

When to arrange an assessment

  • Hip or groin pain lasting more than a few weeks
  • A new limp or pain while walking
  • Pain that keeps you awake at night
  • Stiffness that makes footwear, car travel or sitting difficult

Warning signs that need urgent care

  • A fall followed by severe hip or groin pain, or inability to stand or walk
  • A leg that looks shorter or turned outward after a fall
  • A hot, swollen hip with fever

Go to the nearest emergency department or call 112 if you have severe pain with a deformed limb, numbness, a cold or pale limb, or heavy bleeding.

Accurate diagnosis. Appropriate treatment. Better movement.

The right treatment depends on the cause, severity, duration and functional impact of your symptoms. Consultation helps determine whether you need conservative care, further investigation, rehabilitation or referral for additional treatment.
