Upper limb problems: what patients usually notice

  • Shoulder pain while sleeping, or pain when lying on the shoulder
  • Being unable to lift the arm or reach overhead; a shoulder that feels stuck
  • Pain while combing hair, reaching behind the back or lifting a bag
  • Elbow pain when gripping, lifting or turning a door handle
  • Wrist pain after a fall on an outstretched hand
  • Numbness or tingling in the fingers, often at night, or a hand that feels clumsy
  • A finger that locks or catches when bending, or hand stiffness

Medical terms you may hear

Frozen shoulder (adhesive capsulitis) is stiffness and pain of the shoulder capsule. Rotator cuff tendinopathy and tears affect the tendons around the shoulder. Shoulder impingement is pain when tendons are pinched during overhead movement. Tennis elbow (lateral epicondylitis) and golfer's elbow (medial epicondylitis) affect the tendons at the elbow. Carpal tunnel syndrome is pressure on the median nerve at the wrist. Trigger finger is a catching or locking finger caused by a tendon sheath problem.

Common causes

  • Age-related changes in tendons and joints
  • Repetitive use at work, in sports or at home
  • A fall or direct injury, or a shoulder that has dislocated before
  • Frozen shoulder, which is more common in people with diabetes
  • Carpal tunnel syndrome, which is linked to repetitive use, pregnancy, diabetes and some other conditions

What a consultation includes

  • A history of the pain, the activities that trigger it and any injury
  • Examination of movement, strength, stability and the nerves in the arm and hand
  • X-rays for bone or joint problems where indicated
  • Ultrasound or MRI when they will help decide the treatment
  • Nerve tests where carpal tunnel syndrome needs confirming

Treatment options

Many upper limb problems improve with a structured exercise programme, activity changes, ergonomic adjustments at work, and pain-relieving medicines as prescribed. Night splints can help carpal tunnel symptoms. Injections are used in selected cases, for example for frozen shoulder, tennis elbow or trigger finger. Surgery is considered for specific conditions, such as rotator cuff repair, carpal tunnel release or trigger finger release, when non-operative treatment has not worked.

When to arrange an assessment

  • Shoulder, elbow, wrist or hand pain lasting more than a few weeks
  • Stiffness or weakness that is slowly getting worse
  • Numbness or tingling that wakes you at night
  • Pain that makes it hard to work, drive or care for yourself

Warning signs that need urgent care

  • A shoulder or elbow that looks out of place after a fall
  • A wound over a joint or bone, or bone visible through the skin
  • A cold, pale or numb hand or arm after an injury

Go to the nearest emergency department or call 112 if you have severe pain with a deformed limb, numbness, a cold or pale limb, or heavy bleeding.

PATIENT CENTRED CARE

Accurate diagnosis. Appropriate treatment. Better movement.

The right treatment depends on the cause, severity, duration and functional impact of your symptoms. Consultation helps determine whether you need conservative care, further investigation, rehabilitation or referral for additional treatment.
